March 15, 2017 Director of Programs (Full Time)

Under the general direction of the Executive Director, the Director of Programs directly plans, implements, supervises and evaluates the programs and personnel providing support services to families and individuals at Alameda Point. The Director of Programs ensures that our programs meet the needs of the residents, and achieve the goals set out by APC’s mission, strategic plan and funders. The Director of Programs will provide effective and inspiring leadership, as well as stewardship, implementing and leading a continuous quality improvement process throughout the program and service areas, focusing on systems/process improvement and evaluation. The Director of Programs will promote regular and ongoing opportunities for residents and staff to give feedback on program operations and create forums to encourage resident participation and leadership.FULL DESCRIPTION>>

March 15, 2017 Support Specialist (Full Time)

Under the general direction of the Director of Programs, the Support Specialist provides life skills and wellness based case management and supportive services to APC resident families. The Support Specialist conducts intake and needs assessments, develops individualized family wellness action plans with assigned families, maintains supportive family support relationships, brokers and coordinates with internal and external providers to meet comprehensive needs. The Support Specialist is accountable for ensuring timely and consistent completion of all program service documentation. As a member of an interdisciplinary support team, the Support Specialist is expected to engage residents to participate in wellness-based supportive services that facilitate housing retention and increased self sufficiency. Support Specialists work as a partner with the resident community to develop and implement individual and group service activities that support the creation and maintenance of a strong APC resident community.FULL DESCRIPTION>>

March 15, 2017 Workforce Development Support Specialist (Full Time)

Under the supervision of the Education & Employment Manager, the Workforce Development Support Specialist (WDSS) provides case management focused on life skills, education and employment goals with APC resident families. The WDSS conducts assessments, develops individualized services plans, provides one-to-one coaching, facilitates group activities and coordinates with internal and external providers. The WDSS is accountable for ensuring timely and consistent completion of all program documentation. As a member of an interdisciplinary support team, the WDSS is expected to engage residents to participate in supportive services that facilitate housing retention and self-sufficiency.FULL DESCRIPTION>>
